After clicking on the button, you will be asked to complete a short questionnaire about your medical problem, and you will then be offered a suitable appointment with the relevant clinician. This might be by telephone or face to face.

The appointment will be sent to you by email so please make sure you have given us your email address.

If you are unable to complete the online form or have not yet given us your email address, please ring the surgery on 0116 2766605 to speak to a Patient Services Advisor.

They will then take you through a few questions about the nature of your problem and your request will be passed to the duty doctor for triage.
